You would like to develop a web-based application for patient appointment. The application will allow a patient to make doctor appointment online. For making an appointment, a patient must provide information, such as name, citizen ID, Hospital number (if available), clinical signs and symptoms (to send the patient to the appropriate clinic).
This web-based application requires a computer network and server to operate. However, there is only one IT officer in your hospital, mostly working on fixing computers. Your hospital does not have its own server yet. You propose to the executive board that cloud server should be used for this project. So, you must provide information to the executive board.
1. Why would you choose cloud server, rather than physical server?
2. What kind of cloud computing service model would be most appropriate (SaaS, PaaS, IaaS)? Why? -

From the situation in the hospital have only one IT officer in your hospital and mostly working on fixing computers and hospital does not have its own server then cloud server is very important to reduce costs of employing IT officer, network system, high performance server computer, flexible for maintenance, support, develops app design and high level of server security and privacy because there is personal information.
2. What kind of cloud computing service model would be most appropriate (SaaS, PaaS, IaaS)? Why?.
Hospital should select PaaS cloud computing service because PaaS is a service model in which customers rent for visualize hospital servers and providing platform services for users such as software developers at work and applications, with cloud service providers providing the necessary software and application development needs. I’d choose cloud services for the reason that our hospital as the same as others are cutting off non-medical related budgets as well as to minimalize our IT resources and spending.
So I will make the doctor’s appointments as easy as possible and the patient can do it anytime and anywhere like they can go on a web browser on their mobile and do it.
The type of cloud services that I’d choose will be “SaaS.” The Saas does nor requires our staff and patient to install any application, and it is easy to use. We only have to study how to manage the dashboard and how to be always kept in touch with the SaaS services provider. I think our hospital executive can get how easy it is to use the services and how saving it is. The only thing that I have to prepare myself is the safety and privacy of the patient’s data. so I did some quick research to find out the right services provider. I picked up “Slack” for an example. I went to their website, searching for “Privacy and Terms” then sections about Privacy and Safety. There are tons of information on how they treat protected health data, how they keep the data, how they disclose the data and even how they will manage in case of disaster and data breaches. In the security practices, they also mentioned about standards they adopted to comply with their services, and they are a HIPAA compliant. I think every type of cloud or even traditional data management system has its own risk. The most important is how we manage our system and how we always are prepared for any worst-case scenario.

ฺThe hospital don’t have its own server. If we buy server we must deal with the congestion problem from many patients and staffs connect to it at the same time. So It would be hard investment for the specification of server and network to support connection sufficiently. Moreover, one IT officer has regularly workload to maintenance all of computers in hospital, he maybe too busy to handle more of the server.2. What kind of cloud computing service model would be most appropriate (SaaS, PaaS, IaaS)? Why?
I would like to use SaaS, if there is some specific software in the cloud that can developed both the web-based application for appointment and could storage data such as pdf, text, and image file from the patient uploads. But If there isn’t have that specific software, I will choose PaaS instead. To decide what kind of cloud service to use is based on simplicity first.-

Hi Ohn! I believed SaaS can surely support any media file. They can offer any customized features we want. The only problem that sounds a little scary for anyone who doesn’t know about how we can control the services is that the services provider manages everything. We, as their customer, is called “user.” They charge for like ten somethings dollar per active user. Users can be physicians, IT staff, or others.

Ohn, I found Slack is interesting, but never has experience using cloud as IT/Informatician. It looks easy and a lot of features. As said in my discussion, they seems to care much about data safety and privacy. They also have many customer support channel. Their business survival depends on reputation, I don’t think they will take risk for not do what to do to keep all safe. Bu the way, I don’t mean this is best choice for healthcare or else just may be for to get to know Saas.

I would consider a distributed cloud server, rather than a single physical server, in case a computer server fails. Maintenance would also be less a problem.
2. What kind of cloud computing service model would be most appropriate (SaaS, PaaS, IaaS)? Why?
Either PaaS or IaaS could be appropriate, as we don’t have an infrastructure yet, but it depends on the availability as well. IaaS would be nice if they can speak Thai and understand Thai problems.
PaaS could be good, but it must also provide the software, to ease the starting up process.
Indeed, the problem here is the confidentiality and the privacy of the data, which could make SaaS a more attractive choice…

1. Reason to choose cloud server over physical server is cost saving.
We don’t have to worry about infrastructure, network, storage and maintenance. Initial cost for installation of physical is very expensive but cloud server is scalable and coast saving , affective as it can subscribe as pay per uses or per users. To maintain a physical server with one IT person would be very difficult and insufficient.
2.To choose a cloud computing service, I prefer SaaS. Other services need software developers to developed own web application and deployed that on the services. SaaS provide everything including software, network, storage and security. It may cost more than any other services but hospital staffs don’t need to worry about the software maintenance and problems therefore can concentrate on clinical management.-

Hi Pyae Phyo Aung! Saas subscription is around, for example,15 dollar / active user / months. It’s actually getting cheaper. If ten staff are assigned to use the services, it will cost 150/a/m or 1800 dollars a year or cost of a two PC set with good spec. For a small hospital, an all-inclusive service like this is a peace of mind as they can control all year long budget — no worry about a budget proposal for new storage or server in mid-year.

Hi Ohn ! I checked out some provider and realized that the pricing is crazy!!! I think it is like a playground for developer, so to play you have to pick up your tools and toys for your game. The more complex game the more toys you wanted and the more money you have to pay. One characteristic of cloud service that I like is that it can be scale-up. You can rent cloud storage & service per amount of work that you expect. Once the system is getting bigger or has more users, you can scale it up very easily.

Hi Thongchai! PaaS is attractive for bigger hospitals, especially if they have a lot to data like teaching hospitals or central hospitals. They can manage their PHI which more complex. They can also develop any new application to meet their needs in the research data and information or to manage their specialized decision support. The benefit is that they will not have to install new hardware or software to create or test new applications. The “platform services” can offer them all of that. It will save them time and allowing them to be more agile at work. They can moves thair organization faster with minimalized IT burden while still be innovative org.

I’d rather choose cloud server rather than physical server. According to the limited IT resource in this hospital, I think putting more jobs and complexity of servers will not appropriate. Furthermore, the cloud server has an advantage on reduced IT costs, minimized time to initiate a new system, minimized the workload of server maintenance, and also updating security safeguards of data.

The kind of computing service model should be “SaaS” because of the limited resources of IT personnel in this hospital. SaaS will provide everything that they need, just connected the database and then SaaS will do everything including the application, data storage and management, infrastructure to support, and maintenance of the systems. However, privacy and security of data should be the top-list priority to be concerned. -

I would implement the project on the cloud server because of a couple of reasons. Firstly, the most important reason is the lack of IT staff, only IT staff could not develop the on-premises server effectively. The cloud services could bear the risk of maintaining better such as data back up and data security. Secondly, We might not know exactly how many patients or customers will use this appointment service, thus we can adjust the capacity of the cloud server according to services. The cloud server would save a total cost of implementation. However, one consideration is the data privacy of the patients, thus we might choose the private cloud server rather than the public one.

On the current situation, we would choose the Platform as a Service model (PaaS) rather than the rest due to a few rationales. We have our own system and workflow in the hospital thus we could develop the software that fit our existing system on the PaaS model. However, SaaS cannot provide us with the ability of software development and IaaS is complex for only staff to maintain.-

Most of us choose cloud server and SaaS. We can choose private server for data security. There is one problem in our country that we are not allowed to store data (individual/identifiable patient records) in a database/server which located outside of our country. Is there any such issue in your experience.

Both of cloud server and privacy server have Pros and Cons, however, for this situation, the cloud seems to be the better way. The first reason is the hospital has only 1 IT staff so it is really difficult to handle everything. Although the private cloud tends to be a good choice if we concern security, the cloud server has many layers of protection that we can design for a certain type of threat; and, the important thing is that we need to select a reliable company service. Another reason, I agree with Ajarn Saranath that storage can be added as needed, so if the hospital has enough budget they only pay for what they need.2.
