if I’m going to design an inter-hospital health information system architecture then I need:
1. Health workers (doctors, nurses, pharmacists, medical record officers, etc.) each of whom are responsible for providing input in designing data and information needs in each part of the hospital.
2. Databased administrator who will design the database and the relationship between data from the required variables
3. Network administrator in charge of designing network requirements for connectivity within the hospital including connecting medical devices into the network and connectivity between hospitals
4. Web designers are tasked with making the interface more attractive and user-friendly
5. Security specialists are responsible for designing security systems in applications
6. The programmer is in charge of building software and applications
7. informatician as chief executive to coordinate all tasks of each team member-

If I would like to design an enterprise architecture of a health information system among different hospitals within my province, the person that should be involved in the designing process are
-Representatives of each medical profession e.g. physician, nurse, pharmacist, dentist. They need to provide their needs in using and storing information, and how it should be designed to make the system easy to use.
-IT professions that specialize in web designing, UI designing, programming, data security, and data handling to design practical information systems that are easy to use and meets the requirement
– Health organization executive (e.g. hospital directors, head of provincial public health, head of local health authorities) to make an agreement for each sector to connect the information that can be exchange and transfer between the organization.
– Representative of consumers or patients to be in a committee to monitor for transparecy and the appropriate use of health data
– Lawyers or consultants about health information regulation that can give advises about the law and regulation that the system must follow
– Informatitian to co-ordinate and communicate with each person and plan for creating the best health information system

Good point Auswin made on having a lawyer in the team. They will know the legal challenges the project will be facing.
Karina also mentioned having administrative team for managing financial flows. This is important in projecting the expenses and the capacity of the system to integrate architectural plans and buy hardware equipment needed to run the systems smoothly in according to plan.

For developing enterprise architecture of health information system among different hospitals, we need:
1. health professionals, such as doctors, nurses, radiologists, pharmacists, laboratory technicians who mostly involved in patients data and health records
2. administration officers, such as:
– registration officers who take note of the appointments, enter the demographic data for the patients, room allocations
– finances dept who had role in hospital financial management and billings
3. IT professionals whose work scope are divided into:
– IT personnel who develop the network (which network will be used, local or wide area network)
– IT personnel who ensure the security of the system, doing server maintenance
– IT personnel who prepare the software and hardware for the system
4. database manager who develop the structured and standardized database
5. data analyst, epidemiologist who are in charge of analysing the data to give insight and knowledge
6. informatician who will be responsible as the leader of the HIT architecture development
7. hospital management boards as a supervisor for the whole HIT architecture development to make sure that everything is running well -

In terms of developing an integrated provincial health information system bridging different hospitals, I would have to involve people from various sectors such as health/ medical, IT/ computer science, and administration/ governance.
Health/ medical personnel shall provide their inputs to contribute to data standards, and also ensure that the needs of each department be reflected in the system, while IT/ computer science personnel shall be involved in developing and maintaining the database and network, and ensuring security and its interoperability.
Provincial administration and its governance shall contribute to formulating operation procedures, policy and strategy to ensure the efficiency, effectiveness, and sustainability of the system by reflecting the voice of their beneficiaries. -

Person who should be involved in the enterprise architecture designing process are
1. Healthcare professional; as the expertise of each department (e.g. physician, pharmacist, medical technologist, public health officer, etc.) who know that which health data should be involved to serve the system design
2. Administrative officer and clerk; who perform about general administrative support, inventory, paperwork, and financial data.
3. Client or patient; to know about the basic needs and feedback of the client, which facility they need. Moreover, to make the agreement about patient consent and ensure the data security.
4. Executive board; the important person who is responsible for implementing the policy, make the decision about human resource, and budget support.
5. IT professional; within organization and network, to establish and develop the IT system, database management, privacy and security of the system. Also, we need IT consulting firm to implement and maintain the data system.
6. Health Informatician; expertise in health data system and data interoperability.
7. Government sector representative; to monitor the overall process, coverage of the system, law and regulation. -

In the designing process, assume as I am the manager project architect, I will design each person’s role as shown below.
1. The Director and hospital manager of each hospital should be the architect board that evaluates and supervises the entire project.
2. Healthcare workers, including physicians, nurses, dentists, pharmacists, etc., in each hospital, develop the blueprint of integrated information systems and electronic medical records from their requirements with assistant from IT specialists and database administrators. The other role of the front office is to utilize the health information that has been created.
3. IT professional team (programmer, network administer, security specialist) will have a role to create the interoperability between hospital with the assistant of database administrator and the other’s function such as building security environment, designing program and website, and maintenance network.
4. The database administrator will focus on the data to ensure the quality and integrity of medical data
5. Data analysis and researcher will have a role in the analysis of upcoming data for multipurpose. -

If I would like to draw an enterprise architecture (EA) blueprint for various hospitals in my province, the following should be involved in the designing process;
- Provincial governor: to decide an new EA policy for the province, encourage the hospital executives to follow the policy.
- Hospital executive: to design an EA that align with their business goals.
- Health care provider: to list the challenges when the EA is implemented in their settings and to define the problems about health informatic system in their current setting.
- IT professionals: to develop information system and database, data security

If I were to design an enterprise architecture, I would involve the following stakeholders:
1. Policymakers, which include the governor or public officials – They will set the vision of the project, ensure lawful governance and jurisdictions (e.g., privacy, security) of the data that will be collected.
2. IT personnel from each of the hospitals – Each will ensure interoperability of the health systems when connected to a central architecture
3. Health professional representatives – Knowledgeable of the data that can be collected or disclosed
4. Informatician – Serve as bridge between policymakers and IT personnel; ensure execution of project in accordance to plan; construct the enterprise architecture
5. Patient representatives – Since the data that will be collected will be from patients, they have the right to know what data are being collected and analyzed. They will serve as watchdogs on malicious activities (e.g., selling of personal information).
6. Information security experts – Build layers of security to create an impenetrable architecture against hackers who try to steal data
7. Data scientists – They are knowledgeable on how to best interpret datasets from various sources of information -

Different stakeholders should be involved to design an enterprise architecture in the healthcare system. As we focus on standard, secured, and efficient data from different hospitals within the province, there should be an advancement in the interoperability system.
Front line healthcare staffs play a vital role in storing data in the initial phase. Public health experts are in the leadership role , Informatician disseminates the information within the policy and guidelines, Health information Technologists work on security breaches, programming, and other technical aspects of the data. -

These days there are a lot of concerns regarding security breaches, data privacy and there are different policies and government laws to abide. So, to design an enterprise architecture Lawyer should be included. -

To design an EA of HIS among different hospitals successfully is to have the collaboration of different stakeholders as following
1. Healthcare professionals in each field. They can provide what they need in this system to manage the data properly. They can also provide about GUI that is easy to use in the point of the users.
2. Data controller/Lawyer to discuss the legal issues of the system about data privacy according to the law (PDPA).
3. IT officers (programming, data security, networks, GUI ) to build the system according to the team have planned and plan about the system maintenance.
4. Patient can give their advice about their concern to the system, such as data leakage or consent.
5. Health Informatician to collaborate with all members of the team to achieve the goal of building HIS.
6. Data scientist who could provide how to use and analyze the collecting data for further development.

1. Health policymakers and hospital leaders from each hospital: responsible for the approvals for the project, strategic and capability planning, budget and ensure the alignment with local policy and regulation
2. IT professionals:
• To plan, design, and implement the databases system and network
• Ensure the privacy and security of the system.
3. Data management team:
• This may include data administrators, data analysts, data scientists, epidemiologists and informaticians
• To manage/analyze the data, and also ensure data standard, meaning of data system and interoperability
4. Representatives among users
• Representatives of healthcare professionals: cross-functional team such as doctors, nurses, pharmacists, medical information systems, laboratory technicians and etc, to share their insights on necessary data and system design that can be convenient to use in the real practice.
• Administrative officer who involves in data intake directly: to develop the system design from users’ perspective and test the validity of system and data.
5. Project management team:
• Project managers and project leads/coordinators from each site
• To be a contact person for the team in each hospital/setting and manage the workflow-

I agree with you about the representative among users. The system design should be user-friendly and the person that we should listen to when designing is the officers who involve in inputting the data. If the system is not designed base on user’s needs, it would be hard for them to work with and it will be abandoned.

Before designing province-wide informatics architecture. We need to understand key stakeholders of healthcare systems which are public and private sectors.
Public sectors are composed of community and regional hospitals, public health provisional governments such as NHSO, DOH, DDC, HHS, NIEMS, and administrative provisional government.
Private sectors are also stakeholders.Each stakeholder have different specialized personnel as following
– Healthcare professions: Multidisciplinary personnel specialized in their role. These personnels are working with the patient. Providing data into health informatics systems and using data to treat patients.
– IT professions: These personnels are creating, maintaining, protecting enterprise architecture.
– Researcher: collecting and analyzing data by using an epidemiologic method.
– Administrative personnel: operating non-medical work such as policy analyst, financier, lawyer, general administration worker.
– Private sector and NGOs: working as a collective person who may concerned. -

To build a decent enterprise architecture serving all hospitals located in province, there should be a collaboration between multidisciplines team including
1. Healthcare workers, including physicians, nurses, pharmacists, radiologist, people from labs, etc. These people play a major role as users. They are people who either utilize and collect health data. Their really experience will help to build the system.
2. Executive Officer, this must comprise all executive from every aspect such as Hospital Director, Head of each professional team, head of financial, head of epidemiology team, Head of IT team etc. This group of people will be like Board of the EA
3. IT Professional team, including software engineer, Front-end programmer , Back-End programmer,UX/UI designer to be a builder of the system and also be a team that will have responsibility for maintenance the system in the future.
4. Informatician also play a major role in this system. Their role is to manage all about information including structure, flow, governance and also be an essential person to communicate between each group of person.
5. Project Manager play a role of monitor and plan the progression of project.
6. Patient will also important to give their aspect to the team to build proper system.
7. Data Scientist is also important. They can help improving data utilization, analysis that make us get more insight from collected data
8. Lawyer. Nowadays, health data is more concerned especially about privacy. Lawyer can help us design the system that will fit for regulation such as PDPA and other regulations announced in the future.

For interoperability among the public health organizations, enterprise architecture play major roles in order to integrate and standardize the public health operation for ensuring the alignment between operational system and information technology. To design an enterprise architecture of a health information system, many roles of the organization should be involved, such as the following
1. The executives of the public health organization and policy maker: The key concepts of enterprise architecture is vision and systematic planning which executives play major role to lead organization in a proper direction. Not only the vision, but also strategic planning which should according to the operation.
2. Healthcare profession and operational-level officer: Although executive vision is the leading the organization, but in other hand, operational-level officer should be involved in enterprise architecture design because of directly encounter with the real situation and other practical issues.
3. IT profession: Information technology personnel play roles in planning/creating and maintenance the organization system and be important key person in enterprise architecture.
4. Data scientist: Along the information technology, data base is crucial tool for pooling amount of data. Proper management, analysis and data interpretation will affected to organization operation and decision making of the executives.

I agree with other, due to enterprise architecture plays important role in the integration of healthcare resources. To design an enterprise architecture of HIS among all hospitals in my province, we need staffs include:
– Health professionals including doctors, nurses, technicians, etc. -the data from patients will be collected by them and further interpret to the information of individual health.
– Directer of the hospital – to consult an agreement with all hospitals
– Lawyer or adviser – to advise the regulation between hospitals and about all agreements.
– IT professionals including the performance of network, security system, programming. – to support the stability of the system and ensure the privacy of patient’s information.
– Informaticians, to analyze the data collected from patients and interpret to the information.
– Government or local administration, to support the project among all hospitals, in terms of cost and facility. -

– Health Policy maker : A Policy maker is very important to make things aligned
– Healthcare workers include physician, nurse, Pharmacist, technicians and others. They all are Frontline worker who will use this architecture in the real situation.
