KeymasterYou would like to develop a web-based application for patient appointments. The application will allow a patient to make doctor appointments online. For making an appointment, a patient must provide information, such as name, citizen ID, Hospital number (if available), clinical signs and symptoms (to send the patient to the appropriate clinic).
This web-based application requires a computer network and server to operate. However, there is only one IT officer in your hospital, mostly working on fixing computers. Your hospital does not have its own server yet. You propose to the executive board that a cloud server should be used for this project. So, you must provide information to the executive board.– Why would you choose cloud server, rather than physical server?
– What kind of cloud computing service model would be most appropriate (SaaS, PaaS, IaaS)? Why? -

ParticipantI would choose a cloud server rather than a physical server because
– The doctors can access the schedule that the patient makes an appointment in real-time without conflicting with the schedule. For instance, if the doctor has his/her own schedule (either on paper or a computer system) that does not regularly sync with the central system, he might schedule more than one patient at the same time.
– The only one IT officer can works with another task because the maintenance of the system is done by the cloud service provider
– Save the cost of storage, computer server, and software license. The kind of cloud computing service model that is the most appropriate is Platform as a Service. Platform as a Service (PaaS) lets the hospital control the security of the patient data while flexible enough for IT officer to create or deploy applications that suit the need to the cloud without worrying much about the operating system or the hardware configuration.

KeymasterWhen talking with the executives, money is the main interest. You may need to compare the initial and maintenance cost between physical and cloud servers – the cost should include both direct and indirect cost (for example if you need to hire another IT staff).

ParticipantWith regard to the development of the web-based application for patient appointment, I would choose the use of cloud server rather than physical server.
At the briefing to the executive board, I would like to emphasize some of the expected benefits of cloud computing, such as cost saving, scalability, maintenance and support. If you want to build the system in a conventional way, on premises, without an existing IT infrastructure and in-house HR capacity, you would have to make a huge investment on procurement of infrastructure such as servers/ computers, and recruitment of IT officers with proper skills and experience, as well as ensuring proper security measures to protect patients’ individual information, all of which a service provider of cloud computing can provide for a reasonable monthly fee. Another advantage of cloud service is scalability. as the system can be started small and be upgraded later as the data storage is expanded, instead of building a large system in anticipation of future needs.
As for the type of the cloud computing service, I would like to propose Platform-as-a-Service (PaaS), in which the service provider shall provide database, OS, middleware and security measures, in addition to providing hardware.

ParticipantI prefer a cloud server over a physical server because of the limitations of the IT officer. cloud server allows for cloud hosting so that server maintenance is carried out by the cloud company. I will choose PAAS as cloud computing because it is possible to create my own applications and security features are already available. Besides that, the scalability is also high so that if there are more application users, it will automatically scale the application according to the number of users.

ParticipantSince the hospital lacks IT manpower and does not have its own server, developing a network will be very costly. Thus, the solution I choose would be to use cloud server and Platform as a Service (PaaS) model. Using cloud server and PaaS model will be cheaper because they already had the infrastructure that ready to use. As a customer, we can choose the pricing for cloud server that are suitable for our need and budget because they offer subscription-based pricing. Usually with this subscription, we also get maintenance and support service from the provider, so we do not need to recruit other IT personnel for doing that.
Using PaaS model, we do not need to think about runtime, middleware, OS, server, storage, networking because the provider already offers those. We can focus on the application deployment and data. -

ParticipantI think, as we have one IT officer with no experience in servers networking and infrastructure, we have to choose a Cloud server. This will save the costs of hardware needed to establish the physical server(s) and hiring new 2-3 IT officers to run and maintain the servers 24/7.
Also nowadays there are many cloud hosting providers that comply with General Data Protection Regulation (GDPR) and other data-related regulations which make them a preferred choice for military and governmental organizations.
I would choose SaaS as the cloud computing service model, If we compared the build vs buy costs, It is better to pay a monthly or annual subscription rather than hire someone to build the web application which will take a long time in addition to paying for the periodic maintenance.
In this model, we don’t need to pay extra costs for cloud servers or worry about data backup.
Also, we can find a SaaS solution that provides many features, maybe a mobile application and we can launch the appointment application in minutes to hours with no technical skills.

ParticipantI agree on your point that SaaS is the way to go given the situation. Most likely, there is an existing cloud computing company that provides such services. Hence, there is no need to write a separate application (which may be susceptible to bugs).

ParticipantI prefer using a cloud-based system for patient appointment programs. There are more advantages than using a physical(local) server. Type of cloud service model that I prefer is SaaS.
The cloud-based system usually develops and maintains by large IT corporations. So we don’t need to worry about writing a code or keeping a server secured. Some cloud-based appointment programs are well-integrated into office programs. Microsoft booking is a SaaS developed by Microsoft that integrates seamlessly with Microsoft office suites. It usually complies with the privacy and data government.
There are some disadvantages of using a cloud-based system. The cloud-based system often come with subscription-based and price is vary with how many features you want to use. We can’t customized for specific need. (Some service provider have feature for creating mini program such as Powerautomate by Microsoft.)

ParticipantGiven the limitations of workforce and at-hand infrastructures, cloud computing is the most viable way to go. Investing on physical servers and IT personnel will take time, money, and effort – to which the project may face roll-out delays. Cloud computing is cost-effective in the long run since services are already shouldered by another entity. While cloud computing is susceptible to security attacks, companies that provide such services are vying for reliability and reputation; hence, it is expected that most of the time, the service will be up and running when needed.
Since there is only one IT officer, it would be best to choose SaaS, considering the manpower constraints and especially if the project timebound. The IT officer will need not to write the application for the project since it’s already ready for use. The web application will be minimally supervised, and the IT officer can return to his/her duties of fixing computers. This is under the assumption that there is an exact service available and offered by a cloud computing company. Nonetheless, he/she still needs to learn how the service works and troubleshooting in cases of service outage.

ParticipantThe advantages of a cloud server are less expensive cost when comparing with a physical server, not requiring the area and maintenance for a physical server, IT personnel can work other roles, instead of monitoring and maintaining a physical server. But, the cloud server should provide trustable and strongly data security and data privacy.
What kind of cloud computing service model would be most appropriate (SaaS, PaaS, IaaS)? Why?
For the service of the cloud computing model, SaaS is the most appropriate for this situation. The IT officer doesn’t have to build the web-based application from the beginning. The SaaS can provide both the developing application and maintaining its system. The role of the IT officer can be a coordinator with the SaaS company providing information on what service and system design his hospital need, while he can play a role in fixing computers as before. -

ParticipantI will choose a cloud server rather than a physical server because of the many advantages of cloud computing. Firstly, the cost-effectiveness of using the cloud server you must spend some money for the cloud provider; however, compared to a physical server, you will spend more money for infrastructure system such as computer hardware, data storage, maintaining and repairing the system, hiring some IT professional, etc. Secondly, cloud computing allows us to create workstations in the cloud. Therefore, you can work anytime and anywhere, which is very flexible and will increase your productivity. Additionally, resiliency, using cloud computing with a reliability provider, makes you more comfortable when faced with the disaster of recovering the data. Moreover, scalability, which can expand your information and data, is the other advantage of cloud computing. Finally, although there may be some concerns about the security issue, especially patients’ information, using private cloud computing under the supervision of a data security manager is safe enough.
I will choose platform as a service (PaaS) for the computing service mode because you can create an application that suits your work. Furthermore, PaaS already provide basic infrastructure all you need, such as hardware, software, server, storage, etc. However, you still need an IT professional to take care of this system.

ParticipantI would recommend cloud service because it saves money, time and data is easily accessible when in need. The data in the hospital are very sensitive and people are concerned about security and privacy. But in this situation, as we have only one IT Officer I prefer Platform as a Service (PaaS) as data and applications can be controlled by the hospital and the rest can be managed by the providers.

KeymasterThanks everyone for sharing! It seems that all of you prefer cloud server with several supporting reasons. For the service models, both PaaS and SaaS were selected— which may depends on the capability and programming skills of the IT officer in the hospital.

ParticipantI would choose a cloud server, rather than a physical server because of the limited manpower, system infrastructures and cost-effectiveness. Choosing cloud server could provide multiple benefits. First, there is only one IT officer. Using cloud server can reduce workload for the IT officer and provide a validated system that can ensure the security and integrity of the data. Based on the current situation, the cloud server may require less investment and can implement faster than setting up a new physical server. Additionally, healthcare workers can monitor the data on a real-time basis conveniently.
Due to limitation on workforce, the most appropriate cloud computing service model is Software as a service (SaaS) in my opinion. SaaS requires less development and maintenance which is suitable for small size of IT department. PaaS may be considered in the future after the organization has grown with greater size of IT department and IT teams prefer to develop and control the system by themselves.

If there are too much stuff to do, it would have negative effect to both our IT guy and our hospital.

ParticipantThere are several reasons why I prefer cloud server to physical server. First, we are short on IT staffs which able to maintain the server. Second, the hospital server is not established yet hence it costs a pretty penny to build the server. Third, the cloud server provider has their own IT officers to maintain the server and only costs a monthly service charge. Lastly, the cloud server can be accessed anytime and anywhere if you able to connect the internet.
I convince that Platform as a Service (PaaS) might be the most appropriate service model since PaaS allow us to develop the application.

ParticipantAccording to the given situation, only 1 IT officer, I guess that this hospital might be a small hospital and might only have a simple IT system. Thus I think that to implement web-based application on Cloud network is better than on premise server. These following reason should be presented to board.
1. With only 1 IT officer, it quite tough to implement and maintain hospital own server. On premise server is required IT officer who expertise in IT infrastructure to set configuration and design a decent hospital network. On the other hand, Cloud service provider can help advising and setting these stuff.
2. Good IT infrastructure who expertise about cybersecurity is very essential to keep hospital network safe. Since this web-based application will open to everybody outside hospital to access and input their data, it is very dangerous to be accessible by anyone without good security actions. Again, Cloud service providers usually have a good security provided for clients, hospital just have to take care its own chanel connected to Cloud service provider.
3. Another important issue is about money. Using cloud network is very flexible, hospital has to pay just for real resource they use. With only 1 web-based application using not much hardware resources it might be a lot less money to spend to cloud service provide than to buy a decent physical server. Also about software license, eg SQL server, Windows server OS, Cloud service provider has an option for hospital to pay monthly for these license that make hospital does not need to pay a whole much money to buy license. However, I should do some comparison report with more detail both in long-term and short-term about cost to present to board.
Then, in my opinion with 1 IT officer, I would like to choose SaaS to this project. SaaS might not as flexible as PaaS but SaaS is more ready-to-use in both maintaining and security aspects.

ParticipantI would like to propose the executive board that cloud server should be used rather than physical server because;
First, the reason of reducing the resources, since the hospital has only one IT officer, cloud server can reduce the complexity of the system and ease the IT officer’s burden to maintain the system because cloud server is under the care of IT expertise. Cloud server have no limit in data storage, so it is not require any add-on physical resource which can reduce unnecessary cost.
Second, the security of the system, if we choose the reliability host, cloud service provider establishes the security of the system to support the client and the data backup is also available.
Third, flexibility and scalability, cloud server design to meet specific need and we can manage the scale of service plan that fits for our organization.
Fourth, the convenience to use, the authorized person or team can easily access to the system via any support electronic devices. When focusing on the limit of the manpower, SaaS may most appropriate for this project because there is no internal IT professional needed to install and upgrade the software.

ParticipantFor the given situation, a cloud server is suitable for develop the appointment programs because the data would be share on the cloud system and create the link between user(patient) and hospital. And they also have the security system that can make sure the privacy of the patients. Unlike a physical server, which not suitable for develop the app because there are more specifically use in some are such as only in hospital and also unportable data, it need to be collect in the computer of local area.
And SaaS would be most appropriate a cloud computing service model for this situation because it is a software that’s available via a third-party over the internet. So, the data can upload and share among the user and hospital.
Thank you

ParticipantAccording to the given situation, I think cloud server is much better than physical servers in term of shortage of IT staff and you can access to the system everywhere and every time you need.
Reduce complexity in managing servers since they are under the care of IT expertise. And from the reason that shortage of IT staff, SaaS would be proper in this situation
