I would like you to identify and share with the classmates some ethical principals or good practices in your current job or position you need to have to contribute to the control policy of COVID-19 in your setting. (10 marks)

My current job is managing Home Isolation System for stable COVID-19 patients. All four principles of biomedical ethics (autonomy, beneficence, non-maleficence, and justice) must be considered, but I would like to give an example that I think is controversial in this project.
Non-maleficence is an ethical issue that is important. Since home isolation lets the patients who tested positive for COVID-19 with stable conditions stay at home and do self-care with the help of health care providers remotely by telemedicine. This system must detect an abnormality of the patient early in order to protect the patient from delayed treatment, which is considered to be “do harm” to the patient. Another good example of doing no harm is to breach patients’ privacy. Home isolation System works with many health care personnel (often including other sectors such as delivery services to deliver foods and medical supplies) and sharing patient information must be done carefully.

Based on my experience working in the malaria program, several things were done in the Covid-19 situation, including:
1. Provide PPE (Personal Protective Equipment) for malaria officers in the field.
2. Coordination activities are carried out virtually, including the implementation of a malaria elimination assessment for the sub-country elimination program, after the virtual assessment, the assessment committee will go to the field to validate the data and activities, the virtual meeting which is conducted first makes the implementation of the field validation shorter.
3. The training is conducted online and the training is digitized through the e-Learning platform
4. Health promotion activities on malaria are carried out by using electronic media.-

Many diseases are losing attention during COVID-19 so the facts that you are trying to maintain all malaria activities and protecting the staff from COVID-19 are all good working principals…so well done!

Some points mentioned by Fajar were similar when I transferred to a new workplace. They were very particular on the prevention of COVID-19 spread in workplace.
1. Strict enforcement of wearing of facemask when visiting the office.
2. Only visit the office when absolutely necessary. Contact tracing and temperature checks are in place. Physical distancing is strictly observed. Ventilation is present in the area to allow air exchanges from inside to outside and vice versa.
3. Work-from-home has been in place since the start of the lockdown in March 2020. Meetings are held virtually since then.
4. Health promotion via email messages regarding COVID-19, its transmissibility and preventive measures
5. Trainings are held online.These preventive measures have helped in decreasing the instances of transmission of COVID-19.

Due to the surge in COVID 19 cases ,many countries tried their best to mitigate cases with the help of different public health measures considering the Ethical principles.For example,while doing contact tracing people’s autonomy is considered if they don’t want to participate in the interviews .With the implementation of restrictions to non-essentials travelling ,use of telemedicine, use of proper PPE many people are protected from getting COVID 19 which we can relate with Beneficience ethical principle.There were lot of protest against vaccination for COVID1 9 and government opposes with law enforcement which is justice for people to protect from virus.

We have conducted the following measures to reduce the incidence of COVID19 infection, although many of which were mentioned by others;
1. Telework to reduce the chances of physical contact with your colleagues;
2. Wearing facemasks in the workplace and checking body temperature on entering the office;
3. Conducting virtual meetings and trainings instead of organising face-to-face gatherings;
4. Cancellation or postponement of events that require a physical gathering, even though it will cause significant delays in project management -

I have the opportunity to work on the medical information system related to the COVID-19 vaccine. Therefore, I would say I supported the COVID-19 best practices by providing scientific data rather than providing direct healthcare. I think my role meets the ethics principle on beneficence and non-maleficence because we provided clear, correct, and unbias data that would be helpful for COVID-19 patients’ care and maximize patient safety. Also, my organization encourages employees to work remotely, keep social distancing, wear face masks and face shields, and provide vaccine/health check-up/ATK tests to ensure a safe place to work and prevent the COVID-19 spread.

According to my role as a person who work with clinical data, the four principles of biomedical ethics should be followed especially beneficence and non-maleficence principles. We must act in a way that benefits and avoid harm to patient. Moreover, during COVID-19 situation, follow the organization policy such as wearing a mask, keep distancing to others, avoid unnecessary travelling and get vaccination to prevent the transmission of the disease are the regulation that should be concerned.

As a CIO, I took responsibility to manage all IT systems related to COVID19, including vaccine reservation application, vaccination application, self-monitoring application for patient in Hospitel (Isolation ward located in hotel), and also bed management application.
I have to be align with all ethical principles mentioned in the article, ‘independence’ and ‘transparency’ are in particular. I worked with abundant of patients’ data, so I had to work without any conflict of interest. I have to make sure that all data is full of integrity and nobody can change it in bad way. I also have to make the system transparent and can be audited by auditor. Moreover, I use best practice to secure our application to protect any IT threats.

As a surgeon. I had a chance to be a group member to develop the workflow for a surgical patient during the pandemic, especially the suspicious case or confirmed case of COVID-19.
For developing the operating theatre protocol, we must consider the patient’s disease condition and emergency of disease for ensuring that patient will receive treatment in the timely manner, not delay and taking patient at risk. The other issue that we have to consider is the safety of the healthcare personnel, including surgeon team members, scrubs nurse, anesthesiologist team, etc. We have to rehearse the plan to make sure that everyone in the multidisciplinary team has the same agreement and ask for feedback to develop the protocol and make it more practical and more safety. -

My workplace has enforced some strict practices to help control COVID-19 cases:
• Working from home is mandatory, unless the work needs to be carried out specifically at laboratories
• Minimised face-to-face meeting and utilise the online platform to meet virtually
• Mask is required, N95 or doubled mask
• Eating together is prohibited
• PCR negative is required for coming to office if you have just come back from other cities -

In the context of the drugstore, there are some COVID-19 policies to minimize the COVID-19 transmission
1. Always wearing mask
2. The clients must wear a mask before enter the drugstore
3. If possible, open the all the doors and window to ventilate the air and not using air conditioner
4. Always wash your hands after touch the bills from clients
5. Periodically clean the counter with alcohol -

As a general practitioner, which provide medical service to all of COVID-19 patients from asymptomatic to severe COVID-19 pneumonia in Narathiwat province,Thailand. The beneficence and autonomy ethical principle should be implement in current work as a physician who select the most suitable treatment for each patient with autonomy of them. Not only treatment, but also prevention is the good practice in disease control. For example, seek the asymptomatic infected patient in community to prevent an outbreak in local area simultaneously with treatment of infected patients
