I would like to give my view on point 17 from page 343.
It is possible for two different studies to find identical P values for testing the same hypothesis. The results of tests depend on many factors like sample sizes, and study designs. -

CDSS is used by hospitals for a variety of purposes, including alerting, drug control, and ordering tests. Sending reminders is one example they focus on. The majority of health care facilities in my country don’t use electronic health records systems. Due to a lack of IT experts and resources, lack of interoperability, and restricted budgets, CDSS are implemented in few facilities.

In my opinion, The SNOMED CT coding system has better clinical coverage than the ICD when it comes to encoding the meaning in health information and supporting effective clinical documentation. The ICD-10 is primarily used by coding professionals to classify diseases and in reimbursement.

In my previous work, I collected the primary data by observations in the hospital for research purposes. The research was about the electrolyte balance and we used to visit the patients regularly for the observation in the electrolyte balance and data collection. As the visit used to be more frequent it was more time-consuming.

The GDPR (General Data Protection Regulation) and PDPA (Privacy and Data Protection Act ) are both important privacy laws that protect individuals’ rights and help to protect the confidentiality of their personal information. The GDPR is an EU law that protects the rights of individuals whose data has been collected and stored by organizations. We discussed different case scenarios related to the breach of personal data as follows;
Several personal data records from Cork University Maternity Hospital (CUMH) have been found in a public recycling facility that has been fined €65,000 by the Data Protection Commission (DPC).
Data breaches occurred under the EU’s General Data Protection Regulation (GDPR) and consisted of a large number of documents, personal data of 78 people and the special category ( sensitive ) personal data of six of them. Among the data are race and ethnicity, religious beliefs, political views, biometric information (identifying), gender, and sexual orientation. -

Data privacy and security, Trust in AI, Accountability and responsibility Bias are some common ethical principles that should be addressed in the application of Artificial Intelligence.
For the implementation of AI precautionary principle ‘better safe than sorry’ should be addressed according to the Rio declaration and wingspread statement. -

I think young adults use more wearable Technology which can be a factor for an outcome that is contact pattern. So, the confounder is wearable technology as it is associated with the exposure, that is young adult and not an intermediate step between the exposure and outcome.

Case Fatality Rate
The case fatality rate is abbreviated as CFR. It interprets the severity of the disease and as it is the proportion, it is expressed in percentage.
The numerator is the number of deaths and the denominator is the number of disease cases multiplied by 100. -

Due to the surge in COVID 19 cases ,many countries tried their best to mitigate cases with the help of different public health measures considering the Ethical principles.For example,while doing contact tracing people’s autonomy is considered if they don’t want to participate in the interviews .With the implementation of restrictions to non-essentials travelling ,use of telemedicine, use of proper PPE many people are protected from getting COVID 19 which we can relate with Beneficience ethical principle.There were lot of protest against vaccination for COVID1 9 and government opposes with law enforcement which is justice for people to protect from virus.

In Nepal, Government has been providing necessary and emergency healthcare by the primary healthcare centers,health posts with no cost.But due to lack of healthcare workforce, remote geographical locations ,infrastructure and technologies many people are out of reach of these services.To full-fill this gap many private hospitals are increasing day by day but they are of big financial burden for general people.So,to have affordable, accessible and quality healthcare for all ,Nepal Government has been working to achieve the Universal Health Coverage even though there are lot of challenges to overcome.

In my experience,most of the developing countries are still behind in the implementation of health informatics knowledge and skills among the workforce.In Nepal some of the private hospitals and few government sectors have introduced the health informatics in their Organizations.However, lot of challenges have been seen as most of the workforce needs proper training and don’t like the change management.Data security, confidentiality are also some challenges arsing as there were lack of proper protocols to monitor these problems.

During the COVID -19 pandemic, data sharing plays a vital role in the enhancement of public health measures to vaccine development. As there is a lot of manpower, financial resources involved in the data collection, data sharing can mitigate these burdens. However, there are always concerns about data security, privacy, and confidentiality in regards to data sharing. To overcome these problems, standards protocols should be followed while accessing the database, multifactor authentication, encryption of data, regular updates of the software, and others. So, I am in favor of data sharing with standard protocol without the breach in data security, privacy, and confidentiality.

In my experience, the Implementation of EMR plays a vital role in the outcome of the patient’s health. It also makes the life of all employees easier as all data are assessed within different departments. However, it is not easy to start with the new technology. Some employees are excited to adopt the change management others don’t.
I agree with the bad handwriting by the healthcare workers which misinterprets the data. -

As mentioned in the article there are different big health data challenges like missing Data, Privacy and Ethical Issues, and others. With the advancement of technologies in health sectors, there is a lot of progress going on and more to do to overcome the challenges.
Data are missing as clinicians don’t include them as they are of less importance and sometimes the patient doesn’t wish to disclose their information either. So to overcome this challenge, there should be a standard data protocol to include. Privacy, confidentiality should be protected in various ways like data encryption, multifactor security authentication, while dealing with data and review with the ethical board in dealing with ethical issues. -

I agree with the recommendations from the journal regarding dealing with corruption. Leaders, policymakers are the most corrupted in regards to the developing nations. In my opinion, most of them are due to lack of education and few are under the influence of power, nepotism, and money. Many investigations are conducted against corruption but most of the cases are dismissed as they are very influential.
As mentioned in the journal, government health workers arrive late and leave early, emphasis more on private practices. Even though these are not regarded seriously it impacts a lot on the health outcome of the people. -

In my previous workplace, the government had implemented different strategies for the better outcome of health for the underprivileged population. Different stakeholders from the community also participated to aid the government in delivering the information in health promotion, vaccination. But due to the barriers in technologies and infrastructure, the project was not successful. With the advancement of technologies, service delivery in the healthcare field is improving.

In my view, the implementation of high availability technology in the hospital information system( HIS) will benefit a lot in the delivery of the hospital service in an efficient manner by mitigating IT disturbances, decreasing or omitting downtime. I had experienced admin staff trying to access the patient’s history portal and sharing the diagnosis with his colleagues. As we can see from the CIA Triad principle, this is the confidentiality breach by unauthorized disclosure of personal information. After this incident was reported, staff was given proper training, the disciplinary warning was given , a Strong password was reset and two factors authentication was implemented.

As classified in STEEP, the following challenges are expected by the Pandemic COVID 19;
Social- Due to the lockdown, social distancing, and restrictions on different sectors vulnerable people, children are mostly affected because of the Pandemic. Social gathering activities like wedding ceremonies, funerals, recreational activities are limited to a few people. This all affects social behavior and mental health.
Technological- There was a rapid increase in the use of technology due to pandemics in all aspects from surveillance, treatment, prevention, and also in research and information dissemination.
Environmental-There were reports of positive effects on the environment due to the lockdown and restrictions for many days due to pandemics. A good air quality index was reported in different countries.
Economic- Due to the pandemic, there was a lot of economic burden in the business sectors, a lot of people were laid off from their jobs. On the other hand, the government spent a lot of money on the mitigation of the disease, providing vaccines for prevention and surveillance for identification of disease.
Political- The government used different policies and strategies to aware public by properly disseminating the information about the Pandemic, involving different private stakeholders in the testing and vaccines distribution, and regulating the emergency health rule. More resources were implemented. -

While addressing the nation on the COVID-19 both the leaders, PM Lee Hsien Loong and President Trump included six principles of CERC. Most of the time President Trump was focused on the immediate restrictions of the visitors from China and Europe whereas PM Lee emphasized the scientific facts, compared to SARS, and expressed empathy by explaining how the country was well prepared to fight against COVID-19.

I want to share one of my previous experiences working in a remote health center. There were limited staff and resources but the patient flow was quite high. So, the next day the meeting was conducted and I’ve noticed a few things which when changed can improve the facility and the health outcome of patients. A triage system was introduced for the patient and coaching leadership was implemented for staff which yield a positive outcome for both the staff and the patients.

During this pandemic of COVID-19, digital technologies assisted in different aspects of public health from surveillance, contact tracing, prevention, and treatment. For that different software, mobile and computers are used. The initial phase of COVID-19 treatment is mostly focused on supportive treatment and many countries are practicing telemedicine by performing consultation via video conferencing. This helps to reduce the COVID cases by preventing the healthcare facilities from being over-crowded. But in developing countries, there is a lot to be done in capacity building for preparedness which includes the development of infrastructure, proper training, and development of plans.

PHEIC stands for Public Health Emergency of International Concern which includes the reporting of not only infectious diseases but also the chemical agents, radioactive substances to WHO within 24 hours from which there is public harm and spread of diseases internationally. Most PHEIC diseases are notifiable diseases like Ebola, Zika virus, polio, swine flu, and the recent ongoing COVID-19. All these diseases impacted people’s lives internationally and for the mitigation, control and prevention needed teamwork and effort from different countries.

In the surveillance system, the inaccurate data collected will impact a proper diagnosis of the outbreak, analysis, and interpretation of disease and finally impede the control and prevention of the disease. As the data quality is also one of the evaluation indicators for the surveillance system it will affect the health outcome and mislead in the dissemination of the information. It also increases the cost and will not meet the objectives of the surveillance.

Information Technology plays a major role in various steps of outbreak investigations. It starts right from confirming outbreaks, laboratory analysis, hypothesis testing, and finally to ruling out when there is no more outbreak. Like in event-based surveillance information is collected from local media, reports hotline with the use of information technologies. Different software is used in the analysis and hypothesis testing.

In potential case identifications, digital technology like thermal imaging cameras, infrared sensors, and other wearable technologies are used in airports and other densely populated areas. These technologies detect if the patients are having high fever and are for initial screening purposes. Many countries started digitally connected rapid test, Self swab test, and drive through the test for COVID 19.

Surveillance is the essential component of public health informatics that tracks the disease pattern, identify disease outbreak and monitor the impact of the diseases. We should consider both passive and active surveillance for COVID -19. In passive surveillance data are collected routinely after the people approach health facilities for the treatment. Intervening the direct public measures like the Antigen test, contact tracing is active surveillance that helps in detecting and isolating the COVID 19 individuals in contact.
Syndromic surveillance in COVID 19 should be done in populated areas like schools, workplaces as it helps early detection of an outbreak.
Sentinel surveillance helps for detecting the different variants as it requires additional lab tests and is done only in limited sites.
I would like to prefer the reliable health information disseminated via CDC, WHO Data visualization Dashboards, scientific journals, and websites. -

We have to follow Ethical principles and regulations before assessing the data. In this case, the home address, geolocation, phone number, disease status are the sensitive information. Before collecting these data, individuals should be taken consent and properly informed about the purpose and use of the data. The data redaction technique is very important if in case the sensitive data have to be used for the prevention of disease. However, ethical principles and regulations have to be always considered to avoid privacy and security breaches.

As health Informatician, we cannot disclose the patient’s privacy which is a data breach and against the law. There are many cases where the employees are investigated, fined, terminated from the job, and sometimes are put behind the bars for not maintaining the confidentiality and privacy of the data. However, there are some circumstances like communicable diseases which are serious threats to the public, sexual abuse, rape where designated staff can disclose certain information abiding by the law.

In my previous Organization, Electronic Medical Record System was implemented which replaced the paper-based system. As the system was in the initial phase the cost was within the limit but later with load shedding exceeded the budget. On the operational side, there was proper frequent training and good assistance from the IT expert. People were happy as it was reliable, fast, and accurate. Data were secured and easily assessed in different departments. The successful projects in different Organizations were researched in terms of their application and designed accordingly.

In my previous Organization, Electronic Medical Record System was implemented which replaced the paper-based system. As it was a new system for all staff, few were optimistic and others are overwhelmed with the new technology.
In comparing with the ADKAR model for change management, the following are my opinions regarding the implementation of EMR ;
Most of the staff wanted to implement new technology like in other Organizations which signifies awareness. Employees are interested and motivated, proper frequent training was given, there was onsite shadowing with experts, and a feedback mechanism was also implemented for the improvement. This explains the Awareness, Desire, Knowledge, Ability, and Reinforcement for change management. -

In the Decision Support System different alerts, reminders, and notifications are used to aid in decision making.
In my Organization, the reminders are updated on the message board. All healthcare professionals go through the message boards which makes them alert about their responsibilities. Abnormal lab reports, abnormal X-ray findings are also highlighted with different color combinations to bring immediate healthcare attention which helps for the positive health outcome of the patients.
