POWER2DM (Predictive model-based decision support for diabetes patient empowerment)
POWER2DM is a collaborative European project with multiple partners. The aim of the project is to deliver evidence-based personalized systems for type 1 and 2 diabetes patients. Patients can manage and improve their own health and disease, with the help of their physician and POWER2DM systems.POWER2DM covers all key eHealth domains:
Health in Our Hands:
Self-management support system (SMSS) empowers patients to manage their health through a mobile app and wearables.
Patients can set personal goals, track progress, and monitor health data like blood glucose levels, physical activity, and diet. This gives them real-time control and enables informed decision-making.Interacting for Health:
The system includes a shared decision-making dashboard (SDMD), used during consultations. This fosters collaboration between patients and healthcare providers, ensuring that treatment plans are aligned with individual goals, to create the best treatment plans.Data Enabling Health: The system collects data from various sources, such as glucose monitors and wearables. By analyzing this data, it provides actionable and predictive insights to support data-driven decision-making for the management of each diabetes patient’s condition.
How could this project improve the current health practice?
This project was tested by a randomized controlled trial that involved 226 participants from the Netherlands and Spain. One used the POWER2DM, and the other group received usual care. The study lasted for 37 weeks, and the main focusing on changes in HbA1c level. Other areas measured included quality of life, diabetes self-management, and awareness of hypoglycemia (low blood sugar).
The results showed that:
– HbA1c levels is decreased in patients with type 2 DM
– quality of life is increase in patients with type 1 DM
– better self-management in both type 1 and type 2 DM patientsHow this project improves healthcare

The clinical trial showed that POWER2DM significantly improved HbA1c levels in patients with type 2 diabetes and improved the quality of life for patients with type 1 diabetes. Additionally, it enhanced self-management behaviors, helping patients achieve better glycemic control.In conclusion, POWER2DM is an innovative eHealth system designed to help patients manage diabetes by integrating medical, behavioral, and psychological aspects of care. It empowers patients to take control of their health, improves collaboration with healthcare providers, and enhances clinical outcomes. ParticipantHello Wannisa, thank you for sharing “POWER2DM” project with us. It is a great ehealth project that covers all three domains of eHealth. By using this application, users can have more control on their DM with shared decisions by themselves and their healthcare providers. This shared decision making process interests me. In addition to better control of their glucose level, it is good that most people who use the applications lead better quality of life in terms of health.

ParticipantThis is an interesting project. Traditionally, diabetic records have been maintained on paper or through recording apps [not integrated with glucometer]. However, this new approach [app integrated with glucometer] directly transmits measurements to a database with accurate timestamps, ensuring data integrity and reliability. This makes it particularly valuable for research purposes due to its immutability.
Your discussion comprehensively explores the potential impacts of this project on current healthcare practices. I would like to know if this initiative is ongoing or discontinued.-

Based on the paper I referenced, published in 2023, and another paper in 2024 by Ruissen, M.M., Steyerberg, E.W., Huisman, S.D. et al. (2024), which provides critical comments on the assessment of quality of life and the clinical impact of the POWER2DM intervention, I expect this initiative is still ongoing.
