BMI is redundant because it can be calculated from weight and height. Age also can be calculated from date of birth, so they should be removed.

Race should not be an open-ended question. The CRF designer should offer possible answers in choices and let the data collector choose the answer with a checkbox.

Yes, I agree with you. In additional, because the race is very broad answer to develop the appropriate checkbox, developer should consider the objective of the study and data of target population.

Agree with both of you. Check-box should be provided as much as possible. The answer choices should fit to the objectives.

Date of Visit is not defined format that should be defined date format (MM/DD/YYYY).

Great! Time of visit can depend on the purpose of the project whether time of visit is important or not.

I agree with you. The form should specify the unit or provide checkbox to choose unit if there are different sites that use different measurement system.

Agree! All units should be specified: Age, Blood pressure, height, weight

Checkbox for site id should have 2 boxes in case the sites are more than 9 sites. Moreover, a Pregnancy test should add a “not done” box to avoid data missing.

There’re multiple fields that need units:
Age: years or years with months
Weight: kilograms (kg), pounds (lbs)
Height: centimeters (cm), meters (m), feet, and inches (ft, in)
BMI: kg/m^2

Also, the Date, Date of informed consent can be design into boxes for easier writing
for example, Date could be written as ⬜️⬜️/⬜️⬜️⬜️/⬜️⬜️⬜️⬜️ (DD,MMM,YYYY; 05/APR/2021)

There are many places in this CRF should be corrected and improved and I do agree from all of the above comments. I would like to add some part that this CFR should be improved;
– Use “⬜️” for entering the Weight, Height and BMI
– Should specify the decimal for Weight, Height and BMI
– Blood pressure should specify which one is for systolic and diastolic, and used “⬜️” for data entering.

For my opinion I think I should improve this CRF as follow;
1. The site no. could be have 2 digits
2. Date of visit; It could also be specified as DD/MM/YYYY (01-12 for month) or DD/MMM/YYY (Ex. Jan, Feb,)
3. Date of birth or Age; I think just age is fine for this CRF (Include or Exclude criteria if matter)
4. Race; should be 2 check boxes ( Thai and Others)
5. Pregnancy test; insert remark (*only female) or use other check box (not done) by K.Pongsakorn’s comments

No. of boxes for “pulse rate” should be 3 instead of 2, since it can be more than 99. And for other findings, I agree with all the classmates above.

“Age” and “date of birth” is duplicate, one of them should be use.

Be careful when you ask for Date of birth in the CRF. Date of Birth is considered as identifiable data. To protect data privacy, date of birth should not be specified in the CRF, unless it is critical for the study. Only Birth year or Age would be enough.

the consistency of number that represent normal and abnormal in physical examinations should correlate with pregnancy test.
