You would like to develop a web-based application for patient appointment. The application will allow a patient to make doctor appointment online. For making an appointment, a patient must provide information, such as name, citizen ID, Hospital number (if available), clinical signs and symptoms (to send the patient to the appropriate clinic).
This web-based application requires a computer network and server to operate. However, there is only one IT officer in your hospital, mostly working on fixing computers. Your hospital does not have its own server yet. You propose to the executive board that cloud server should be used for this project. So, you must provide information to the executive board.
1. Why would you choose cloud server, rather than physical server?
2. What kind of cloud computing service model would be most appropriate (SaaS, PaaS, IaaS)? Why? -

A cloud server has the advantages of security, access from wherever whenever, savings, scalability, and flexibility.
The cloud computing service model would be most appropriate is PaaS for this project because a hospital wants to develop a web-based application for patient appointments. PaaS has suitable for the application that able to configuration applications.
The developer will be able to control the tools installed within the platform by themselves. And able to create a platform that is tailored to specific application needs As needed.

Sila, as you are a programmer and IT in a hospital in Thailand. I know that you have a good programming skill. Do you think that IT officer in other hospitals (in general) would be able to handle or config PaaS cloud service model?

I think most hospital IT offices might not be familiar with that server. May not be able to set up the system.

To provide information to the executive board, I would like to describe in the following idea.
1. Cloud server has more reliability because this webserver portal has to connect from many clients at the same time so we expected high demand for input and output that will cause conventional server overload easily but utilized cloud server will prevent this problem as cloud server provider deployed many servers around the globe to ensure its stability with technical support to maintain the server 24 hours every day. It also scalable as patient data expanding. However, cloud server has a certain disadvantage that it may not work during internet connection downtime but those issues rarely occurred as modern internet infrastructures are more reliable and we can switch to another Internet Service Provider anytime
2. The type of cloud service model depends on the software that the hospital used. For a small hospital, I recommend using Software as a Service because it is easier to deploy as they provide complete package software that we can use immediately.

In this case, if I will propose to the executive board why we need to use a cloud server for developing patient appointment applications, I would mention that the cost-saving is the main factor to consider because the physical server has a high cost of installation. Moreover, the organization will buy licensed software and other accessories to install the server. Cloud servers reduce those issues including the cost of maintenance and support. Another benefit of the cloud server is security which is essential for storing the identity of patients. In addition, mobility and flexibility are factors that I would propose to the executive board because the only 1 IT officers in the hospital can access and solve a problem anytime and anywhere.
SaaP is suitable for developing patient appointment applications in hospitals because all software and hardware are provided and control by the cloud service provider and it easy to deploy the application. This will help small organizations grow and well-manageable.-

I agree with this comment. SaaS is suitable for the hospital especially almost the public hospitals will have a problem with money. So this one will help the hospitals to reduce the cost and patients will access to make an appointment easily.

I will propose to the executive board that the cloud server be used for this project because it will save costs. This benefit will help the hospital to save substantial costs because they do not need any physical hardware investment. Furthermore, the cloud service provider will quickly update, support, and maintain service. There is unlimited storage and scales automatically. The patients can access easily from everywhere. Also, there is data security that protects patient information. I will use SaaS for this project. It’s suitable for one IT officer who manages and fixes the problem for patients as I mention the advantages above. The cloud service provider will provide everything for the service, which fits with the hospital’s limitations.

I agree with you that SaaS would be suite for this situation; Limit IT officer, save cost and time in develop and maintain the system. But I would like to add the concern in using SaaS which you can see in my comment.

Cloud server is the choice because of available IT resource in hospital was only one IT officer that can not maintenance system. Total cost of cloud computing was cheaper than on-premises and easy to manage. PaaS was selected because of the project was only system of appointment that has 2 component : application and database. This method will reduce time to implementation of the system and reduce cost of maintenance.

Cloud servers can be configured to provide levels of performance, security and control similar to those of a physical server. Moreover, cloud servers can optimize IT performance without the huge costs associated with purchasing and managing fully physical infrastructure.
PaaS would be the most appropriate one for this project because it has resources that allow a hospital IT person to create both simple and advanced apps. In this case, the hospital just want to create a appointment system. It save cost of maintenance and support secure network connections, while the IT person can manage programs and services they create on the platform.

That’s a very good answer, friend. Anyway, for the second answer, I think if you choose PaaS, you would need more than one IT officer to run the web—based application for creating application software and its time to time maintenance and support.

In this case, I will propose the board to use cloud server because of some reasons. The main reason is manpower, this hospital has only an IT officer which will be struggle if we decide to use on-prem server, it hard to established, maintenance, and troubleshooting. Cloud server has less expensive to set up, no need IT officer to maintenance and easily scalable when needed. Cloud server also has high level of security protocol, in both physical and logical if we use hybrid or private cloud providers. I will propose the board to use cloud service as software as a service (SaaS) because of the manpower too. Only one IT officer cannot develop, test, deploy, set up infrastructure, monitor and maintain the service. So, we have to use the package software of cloud provider for making everything runs. It could be some problem in flexibility issue in the software but, there is the better way to initiate the cloud-based application. In the future if we have a larger, expert IT team. We can develop our own application and migrate to cloud service as platform as a service (PaaS) or infrastructure as a service (IaaS) to save cost and more flexibility.

In my opinion Cloud server is the better than physical server especially in saving cost, high security and flexible for IT and users, reduces the risk when starting this project or during project.
and I prefer to use SaaS (Software as a Service) because its saving cost, saving time (for 1 IT) and there are no need for additional investment in hardware and software licenses, as well as system maintenance costs. The server service provider will be the administrator of all systems, the users can access to applications from anywhere via an Internet connection (Mobile phone or tablet). -

The reasons why I would like to choose the Cloud server rather than the Physical server because;
1. We (hospital) have only one IT officer who mostly working on fixing computers. To reduce the support/work from the IT officer, choosing the Cloud system would be suited for this situation.
2. Cost effectiveness: It reduces the expense of procuring and maintaining equipment.What kind of cloud computing service model would be most appropriate (SaaS, PaaS, IaaS)? Why?
I would think about the best 2 models that would appropriate for this situation:
1. SaaS: As we know that we have a limit of IT officer (only one, may not have much time to support/work on this propose), SaaS would be suited because: SaaS is cloud application services provided and managed by the third party/vendor-they will manage all potential technical issues, such as data, middleware, servers, and storage, maintenance. It is reducing the time and money spent on tedious tasks such as installing, managing, and upgrading software. Currently, there are many SaaS applications that available in the market and would answer our requirement.
However, there are some reasons that we would concern;
– Interoperability: Integration with existing apps and services can be a major concern if the SaaS app is not designed to our requirement for integration. In this case, hospital may need to design their own integration systems.
– SaaS apps offer minimal customization capabilities.
– Lack of control: SaaS solutions involve handing control over to the third-party service provider. These controls are not limited to the software; in terms of the version, updates, or appearance but also the data and governance.
– Feature limitations: SaaS apps often come in a standardized form; the choice of features may be covered all needs.2. PaaS: If I am IT office, I would choose the PaaS because PaaS is similar to the SaaS but it allows user/developer to create customized applications, can control and managed their data which would answer the needs.

I agree with you. I have complete coursera : From Idea to startup, in this course has a interview WIX.com that provide SaaS cloud web building template. I think this solution was match to hospital that don’t have IT officer that can create own application.

The advantages of cloud server include accessibility to resources, mobility, scalability and data security of patient. Another reasons are the server can save cost and responsibility of maintenance and support system because the cloud service provider will usually offer ongoing support services. So, It is appropriated server with this hospital that has only on IT officer.
SaaS or Software as a service is cloud computing service model would be most appropriate for this web-base application because not only number of IT worker of the hospital, but I think the application isn’t complex which may more appropriated to use SaaS and not need to use the other models that more complex than SaaS.

1. The cloud server is a better choice over physical server in many ways. Firstly, both consumers/patients and service providers can have access to the appointment application anytime anywhere increasing the service usage by both sides of the users. Since there will be large amount of data used for the application, the cloud will reduce the cost demanded by the physical server, since the former is scalable. Moreover, the cloud service will have network security expert and maintenance experts enabling the existing IT officer to complete his/her job without increased burden and healthcare providers (e.g., medical technologists) to focus on their primary job without additional technological skills finally leading to better patient care. The swift flow of data also ensures efficient and effective appointments.
2. Although PaaS would be the cloud model of choice for hospitals because it allows more customization, I would choose SaaS in this scenario. Since we only have one IT officer, SaaS has benefits by providing quick adoption of the application without excessive costs or efforts. The IT officer will also not have to worry about extra maintenance because only specific functions are enabled to the user. -

1. Reasons why cloud server should be chosen rather than physical server are as follow.
It is cost benefit because we won’t need to arrange increasing infrastructure for better performance of existing IT structure and also less IT technician needed to maintain the system.
Cloud computing can also make business continuity in case of natural disaster or electricity cut off as data stored is backed up by cloud company.
Furthermore, employees can access to data anywhere anytime which can make easy for better communication at work.
2. I would say SaaS is an appropriate model to be used in this situation. Because only one technical staff is present in office and SaaS can greatly support by reducing time spent on installation and upgrading software. Moreover, SaaS can also support for web and mobile applications so that patient can make appointment with doctor more easily and quickly. -

I think PaaS can also have more benefit in long term. Only according to the above scenario, due to lack of manpower and time limit, SaaS is preferable. For me, PaaS should also be considered after some stable condition with SaaS as mitigation to hybrid model as well as integration of web services and database could be done. -

a. As there is only one IT officer at the hospital and his working schedule is tight with fixing computers, if I choose physical server for the web-based application, I would definitely need more human resources.
b. Secondly, as the patients will make doctor appointment online, the server should be on the cloud where the patient from anywhere can access to the server anytime.
c. And using cloud server would save much money, and it would not require maintenance and support.
d. Fourth, the hospital wouldn’t need to consider the storage problem as well. And cloud server scalability is an asset as it allows the hospital to adapt size according to the increase and decrease in volume of data.
e. Data security and backup is one of many things to choose cloud server as the hospital data will be safe and sound relatively in the cloud.2.Given the fact that there’s only one IT officer at the hospital and the requirements of web-based application for patient appointment (providing patient information such as name, ID, Hospital number, signs and symptoms) are relatively simple, I believe the SaaS would be the most appropriate cloud computing service model. Moreover , the roles and responsibility of application software, data, run time, middleware, operating system, virtualization, server, storage and networking would fall in the hands of cloud service provider at an affordable with additional benefits such as flexibility, efficiency and reliability.
