Dear Saravalee
Your CRF is impressive and covers all of the information needed. I would like to comment about minor details to improve the CRF,
– It might be easier for the data entry clerk if you add codes after each answer (eg. Yes(1) No(2))
– For sex, instead of adding “undifferentiated” answer, you could specify as “sex at birth” to avoid confusion, and for “unknown” answer, in my opinion, it might be redundant since it is an important data that could not be missed when collecting data.
– Pregnancy test can be answered “not tested”, so the choices should provide this answer.
– Year of birth should be specified as BE or AD
