I would choose a disease based on Highly-transmissible or High-severity, as well as, less done by other people.
I would consider regional diseases, that may be forgotten by western people, such as Tuberculosis or Melioidosis, as it can possibly be totally eliminated from the country.

Generally, no. Unless I notify the client that the data will be given out, and given chance to participate or withdraw from the project, in order not to violate the Principle of Privacy.
