Some tool bar is not similar to the guideline. So, some order such as Text format (Page No. 12 in practical file) that it not found in my program. Then I cannot set Bold style.

I found that after clicked the .CSV file for attribute table, it did not have the total column like the practical file. Luckily, this column is not used. I just want to know anyone found like me or not? Or I did something wrong.

Please help to improve this CRF. One spot per each student.
Due to there were many points were mentioned, I will suggest other things. In my opinion about the heading “screening and vaccination”, The CRF have to consist of more details such as medical history or the data for receiving vaccine.
