I agree with you. The form should specify the unit or provide checkbox to choose unit if there are different sites that use different measurement system.

Thank you for sharing. I agree that working with the non-original file is a safe way to prevent the loss of original data. But working with copied files sometimes confusing, especially when the database is still open for a new record. There will be many versions of files created and downloaded.

Race should not be an open-ended question. The CRF designer should offer possible answers in choices and let the data collector choose the answer with a checkbox.
