      In my opinion, a well-organized CRF form with all variables included. The questionnaire in the form is easy to understand and the choices given are relevant according to the questionnaire.

      Your CRF is very comprehensive and clear. I like the way you provide the choice of units because each study site may use different unit of measurement.
