Your considered preventive measures were already completed and I just would like to add one more thing. This is securing the disposal of PHI, this would be also one measure if Yakima Valley Memorial Hospital set up the procedure to dispose properly of paper records containing PHI through shredding or secure disposal methods. Ensure that electronic devices are also wiped clean of data before disposal or reuse.
